Is mobile phlebotomy covered by insurance?

Coverage varies by insurance plan and by what is being billed.

The lab tests themselves (diagnostic codes):

Most insurance plans — including commercial insurance, Medicare, and Medicaid — cover the cost of lab tests ordered by a physician, regardless of whether they were collected at a clinic or in your home. The lab submits the claim using standard diagnostic codes.

The mobile phlebotomy service fee (travel/collection fee):

This is a separate charge billed by the phlebotomist or mobile phlebotomy company. Coverage is inconsistent:

  • Some plans cover it under a home health or specimen collection benefit
  • Medicare Part B covers a "specimen collection fee" in limited circumstances (notably for homebound patients)
  • Many commercial plans do not cover it as a separate line item
  • Best practice:

    1. Call your insurance provider and ask: "Do you cover a home specimen collection fee or mobile phlebotomy service fee?"

    2. Get a reference number for the call

    3. Confirm the exact CPT billing code the phlebotomist will use (usually 36415 for routine venipuncture)

    4. Ask the mobile phlebotomist if they are in-network

    When in doubt, ask for a self-pay rate, which many providers offer at a lower cost than the insurance list price.
