What documents do providers need to join the platform?

Providers must submit their phlebotomy certification, proof of professional liability insurance, and complete their profile before being approved.

Required documents:

  • Active phlebotomy certification (ASCP, NHA, AMT, or state equivalent)
  • Proof of professional liability insurance (certificate of insurance)
  • Government-issued photo ID
  • Completed provider profile (service area, pricing, specialties)
  • Optional but recommended:

  • Additional certifications (DOT collector training, pediatric phlebotomy, clinical trial experience)
  • Business license or LLC documentation
