Who is responsible for redraws?

Responsibility depends on the cause. Hemolysis from poor technique falls on the provider. Patient factors such as difficult veins are generally shared responsibility.

Provider responsibility: Hemolysis from rough technique, incorrect tube type, inadequate mixing, improper temperature handling during transport, mislabeling

Shared/patient responsibility: Hemolysis from fragile veins, dehydration, or patient medication effects

Lab responsibility: Processing errors, equipment malfunction, shipping delay that caused stability failure (if provider shipped correctly)

For provider-caused rejections, redraws should be offered at no additional charge to the patient.
